Suspicion of cancer Rapid Access Clinic

This clinic provides a one-stop urological assessment for patients meeting the National Institute of Clinical Excellence (NICE) (2004) referral criteria for urgent suspected cancer referrals.

On the NHS this criteria states patient will be given an appointment within two weeks of receiving the appropriate GP referral. At London Urological Practice, we are able to offer patients appointments within two days.

Our Rapid Access Haematuria and Prostate assessment Clinics is an outpatient diagnostic service, which ensures patients are offered essential investigations on their first visit in order to reduce multiple hospital visits and thus the time period to a definitive diagnosis.

This diagnostic service is in line with national guidance and consistently offer a rapid response time well within the national waiting time targets i.e. from date of GP referral; patients are offered an appointment within 2 days, compared to the 14days offered on the NHS.

Patient's views on their experience at the clinic are regularly sought and the results have influenced how the service has developed over the past few years. This has led to a high level of patient satisfaction with the service they receive.

The Challenge identified

Previously all haematuria referrals were seen following referral in an outpatient clinic and investigations requested from this point. The investigations were all standard; the outpatient clinic review added little to the process and nothing that could not have been achieved without this step. Commonly patients were reviewed between investigations and then moved on to the next investigations, a process that added unnecessary complexity and time and only rarely changed the pathway.

The plan is to streamline the investigations, cut out unnecessary steps while keeping supervision and maximising benefits.

One-Stop Clinics

The concept of a "one-stop" clinic is that all of the tests that your condition requires are done on the same day. While this takes longer, it means that you are less likely to need to return for various investigations on another day, and you are more likely to have a diagnosis on the same day as the clinic. Dependent upon the findings of the tests, though, you may still require further treatment, and another appointment would be required for this.

We aim to offer a one stop service for patients with haematuria (blood in the urine), for patients with a raised PSA or suspected prostate cancer, and for patients with lower urinary tract symptoms.

Haematuria "One Stop"

This clinic runs with the aim of comprehensively managing patients who have had haematuria (blood in the urine). Where possible, all investigations required to make a diagnosis, including appropriate radiology and flexible cystoscopy will be performed at the same visit, thus allowing the majority of patients to go home knowing their diagnosis.

Prostate Diagnostic Clinic "One Stop"

This clinic runs with the aim of comprehensively managing patients who have a raised PSA or abnormal digital rectal examination. Where possible, all investigations required to make a diagnosis, including prostate biopsy (if appropriate) will be performed at the same visit, thus speeding up the diagnostic pathway and decreasing patient anxiety.

Lower Urinary Tract Symptoms "One Stop"

This clinic runs with the aim of comprehensively managing patients who have lower urinary tract symptoms. Where possible, all investigations required to make a diagnosis, including urine tests, flow rate and post void residual and flexible cystoscopy if appropriate will be performed at the same visit, thus allowing the majority of patients to go home knowing their diagnosis and with a treatment plan commenced.
